An IME, or independent medical examination, is an evaluation by a physician who has not treated the patient, used across workers' compensation, disability and liability matters nationwide. A QME is California's statutory version of that role: a Qualified Medical Evaluator certified by the state and assigned through the panel process. What is the difference between an IME and a QME?
The difference is appointment and jurisdiction, not clinical purpose. An IME is the general term for an examination by a physician who is not the treating doctor. A QME is a California-specific evaluator certified by the state's Division of Workers' Compensation and assigned to a case through a panel. Outside California, the same examination is normally just called an IME.
Is a QME an IME?
Functionally yes. A QME evaluation is an independent examination, so in plain language it is an IME performed under California's statutory framework. The distinction matters for paperwork: a QME report follows California forms and timelines, while an IME report follows whatever the requesting jurisdiction or carrier specifies.
Who appoints an IME and who appoints a QME?
An IME is normally arranged by a carrier, employer, third-party administrator, attorney or IME management company. A QME is selected from a panel issued by California's Division of Workers' Compensation, which is why QME scheduling and timelines are more tightly prescribed than general IME scheduling.
What does a QME report contain that a general IME report does not?
California-specific structure. QME reports typically address Permanent and Stationary status, whole person impairment, apportionment under Labor Code sections 4663 and 4664, and are often delivered on PR-4 or narrative formats. A general IME report in another state answers that state's questions instead, such as New York's IME-4 form or Ohio's allowed-condition percentages.
Does IME and QME transcription differ?
The audio is handled the same way; the output is not. QME dictation has to land in California's report structure with the impairment and apportionment language intact. IME dictation from New York, Florida or Ohio is formatted to those systems. That is why we keep separate templates per jurisdiction rather than one generic medical template.
What is a PQME and how does it relate?
PQME is common shorthand for a panel QME — the QME selected from the state-issued panel. When people search PQME vs QME they are usually asking whether it is a different credential. It is not: it describes how that QME came to be assigned to the case.
