What changes from state to state?
The report a California QME dictates is not the report an Ohio BWC examiner dictates. California uses QME and AME panels and PR-4 reports; Ohio examines allowed conditions; Florida uses major contributing cause and Expert Medical Advisors; New York uses IME-4 and IME-5 forms and schedule loss of use.
